New developments in clinical nutrition

On November 23, 2024, the annual clinical nutrition symposium took place at the UCLouvain Brussels Woluwe site. It was organized by the Comité Liaison Alimentation Nutrition (CLAN) and the Food and Dietetics Department of the Cliniques universitaires Saint-Luc. The event brought together more than 200 participants and was highly successful. Six presentations were delivered by expert dietitians, nurses, and physicians, covering a wide variety of topics such as contaminants in our food, new nutritional challenges for cystic fibrosis patients, hospital meals, dysphagia, refeeding syndrome, and celiac disease. The symposium was characterized by dynamic interactions between speakers and team-based presentations, making it a particularly engaging event. In this manuscript, we briefly summarized each presentation along with the names and roles of the speakers.

ANCA-Associated Vasculitis: State of the Art and Perspectives

Anti-neutrophil cytoplasmic antibody (ANCA)- associated vasculitides are characterized by necrotizing inflammation of small blood vessels and primarily affect the kidneys, the lungs, and the upper respiratory tract. These conditions mainly include granulomatosis with polyangiitis and microscopic polyangiitis. Diagnosis relies on serological detection of ANCA directed against myeloperoxidase or proteinase-3, and on biopsy of an affected organ, most commonly the kidney. Management is provided by specialized centers with expertise in ANCA-associated vasculitides. Induction therapy for ANCA-associated vasculitis with kidney involvement or life-threatening manifestations consists of a combination of glucocorticoids and either rituximab or cyclophosphamide. Rituximab is preferred for relapsing cases. A rapid tapering of glucocorticoids, following the PEXIVAS trial regimen, is currently recommended to significantly reduce the risk of severe infections without compromising efficacy. For remission maintenance, fixed and repeated doses of rituximab administered over 18-48 months are preferred over azathioprine. The recent identification of the role of the complement system in the pathogenesis of ANCA-associated vasculitides has led to the development of avacopan, an oral C5a receptor antagonist. Administering avacopan alongside rituximab or cyclophosphamide for one year has demonstrated superior efficacy in achieving remission and kidney recovery, with lower toxicity compared to glucocorticoids, thus representing a promising therapeutic alternative.

Aeroallergen mapping in a pediatric pneumo-allergology consultation

Each pediatric pneumo-allergology consultation begins with a thorough medical history, followed by a complete clinical examination. After these preliminary steps, additional investigations may be requested. Records of 993 patients who attended the pediatric pneumo-allergology consultation at the UCL Namur university hospital (Godinne site) during the first semester of 2023 were reviewed and analyzed. Based on the medical history and symptoms, skin prick tests were performed for aeroallergens using the prick test method. Skin prick tests were conducted in 204 children between 7 months and 17 years and 3 months of age. Among them, 102 showed positive results. The distribution of sensitizations revealed a clear predominance of sensitization to house dust mites (58.8%), followed by cat dander (37.3%) and grass pollen (33.3%). Polysensitization was observed in over 60% of sensitized patients. In the management of pediatric respiratory allergies, allergen avoidance is the primary step, with a particular emphasis on environmental management. In the case of a positive test for an animal, avoidance remains challenging, even if a correlation between symptoms and the test result has been well-established and recognized by the family. Avoidance could not be achieved in the immediate environment in 68.4% of cases for cats and 66.6% of cases for dogs.

Gender dysphoria in minors and young adults is an increasingly discussed issue. This subject, which is often poorly understood by the general population but also by many health professionals, tends to divide people. Its treatments, in particular, are the source of opposing views. The law against conversion therapies that passed in Belgium in 2023 has made it even more complex to treat these young people. Gender dysphoria can cause significant psychological distress, and physicians have a duty and should be given the opportunity to diagnose the condition and propose treatment to help patients recover their health. Physicians’ clinical judgment and freedom to exercise their medical function should not be subject to interference based on ideas or beliefs that go against the patients’ well-being

Fractures of the proximal femur are common in patients aged over the age of 75. These fractures are associated with significant mortality and morbidity, which drastically increase with surgical delay. There is a marked rise in mortality when surgical treatment is absent or delayed, and there is generally no reason to postpone surgery, as it puts patients at greater risk. This article aims to inform general practitioners, geriatricians, anesthesiologists, and emergency physicians about the different types of femoral fractures, their effect on femoral head vascularization, and the importance of timely surgical management within 36 hours. By involving non-surgical healthcare providers, we hope to reduce surgical delays and improve clinical outcomes for patients with these fractures.

Tirzepatide (Mounjaro®): a new tool in the treatment of obesity

Tirzepatide (TZP) is the first dual agonist, targeting both GLP-1 and GIP receptors, developed for obesity and type 2 diabetes mellitus treatment. TZP is more potent regarding glycemic control and weight loss than GLP-1R mono-agonists (GLP-1RAs) already on the market. Weight loss results almost exclusively from caloric intake reduction. TZP also exerts beneficial effects on the different parameters of the cardiovascular risk profile. TZP is characterized by a good tolerance and a satisfactory safety profile, comparable to that of GLP-1RAs. As with GLP-1RAs, its use raises many questions (cost and possible reimbursement, duration of use, very long-term safety, place in relation to lifestyle and dietary treatment, place in relation to bariatric surgery, etc.). Although all of these questions deserve to be addressed, TZP represents a new step in the development of increasingly effective drugs to treat obesity and its comorbidities.
